Yampa Valley Medical Center is committed to providing safe, personalized and quality services to all patients, regardless of their ability to pay. YVMC’s Financial Assistance Program and Colorado’s CICP program can provide relief to patients and families to ensure that they receive the healthcare they need.

You may qualify for financial assistance with or without health insurance. In the event that you do not qualify for one of our programs, YVMC can help you with your medical bills by setting up a payment plan.

How to get started:

Take three minutes to fill out our online questionnaire, or print out the printable questionnaire and return to YVMC.

You will be notified within one week whether or not you should apply for financial assistance.

If you fit within the guidelines

You will begin the application process. You will receive a checklist to help guide you through the process. The application process can take up to six weeks.

 

Please note: All forms can be filled out by a family member, friend, employer or co-worker in order to better assist the patient with receiving the relief they need.

What if I do not fall within the guidelines but still need assistance?

You will be referred to our billing office to setup a payment plan. Payment plans that are 10 months or less are interest free. Plans that are 11-36 months are at eight percent interest.
